    As we celebrate Mother’s Day, let’s recognize the critical role Medicaid plays in supporting maternal health and advancing equitable access to care for millions of women and families across the United States.

    Medicaid is a major source of health coverage in the US. It advances health equity by ensuring that people living with low incomes can get the care they need.

    In 2023, 16.2 million women aged 15-49 were insured by this vital program, accounting for 21.4% of all US women of reproductive age.

    Medicaid also covers essential sexual and reproductive health services, eliminating what might otherwise be significant cost barriers to care. For example, pregnant people enrolled in Medicaid receive coverage for prenatal care, birth and at least some post-natal care.

    All parents and caregivers deserve access to high-quality SRH care, regardless of where they live or their socioeconomic status.

    It’s Free The Pill Day! One year ago, the first over-the-counter birth control pill—Opill—became available across the US. This was a landmark achievement for reproductive freedom thanks to 20 years of work from Free The Pill coalition members and many others.

    Bringing Opill over the counter has been an important part of efforts to increase contraceptive access for marginalized communities, including Black, Brown and Indigenous people; people living with low incomes; young people; LGBTQ+ folks; people with disabilities; immigrants and more.

    But the work isn’t over yet. We must keep fighting to ensure that Opill is accessible. That means fully covered by insurance, with low and no-cost options, and stocked nationwide without barriers.

    The suspension of Title X funding has completely eliminated care provided by this vital program in 7 US states and partially eliminated care in an additional 15 states.

    If this withholding of funds becomes permanent, more than 800,000 patients nationwide will lose access to Title X-supported care over the course of a year.

    In the first few months of his second term, President Trump swiftly removed Biden-era protections for reproductive rights, issued guidance to reinstate the global gag rule, terminated all US family planning grants, slashed funding to Title X, and more.

    On April 21, a judge in Wyoming issued a preliminary injunction to block two restrictions that targeted the state’s only procedural abortion clinic and forced it to stop providing care.

    Now that these restrictions are blocked, Wyoming’s only procedural abortion clinic @wellspringhealthaccess can resume providing care.

    A decision from the state Supreme Court on whether or not these restrictions can be enforced is expected at a later date.

    Our interactive map is up to date, and Wyoming remains “Restrictive.”

    Title X was specifically designed to address disparities in access to sexual and reproductive health care and to promote health equity.

    This includes serving people living with low incomes, Black, Latinx and other marginalized communities who already encounter systemic barriers to health care.

    The suspension of Title X funds will disproportionately impact the very same communities.

    Several of the Title X grantees that lost funding are @plannedparenthood affiliates across the nation. It’s clear the anti-abortion movement is once again targeting this critical provider of comprehensive sexual and reproductive health care.

    This is unacceptable.

    Guttmacher stands with Planned Parenthood because we believe that birth control, STI testing and treatment, and cancer screenings should be accessible for all.

    A bill moving through the Texas legislature (SB31) would further erode abortion access in the state if allowed to go into effect.

    This bill claims to “clarify” exceptions to the state’s total abortion ban, but it could expand the state’s power to prosecute anyone who helps someone get an abortion—including support organizations like abortion funds and possibly even patients.

    The current total ban is forcing people to leave their home state for basic abortion care. Guttmacher data show that over 34,000 Texans were forced to travel out of state for an abortion in 2023.

    Texans deserve access to abortion care in their communities. This bill is a deceptive measure that would allow for the possible criminalization of patients and support organizations.

    On #WorldHealthDay this year, more women and newborns are at risk of preventable deaths than last year, due to US cuts to foreign aid.

    Our estimates show that in low- and middle-income countries, meeting all needs for sexual & reproductive health care in one year could prevent 76 million unintended pregnancies and 26 million unsafe abortions, resulting in 186,000 fewer maternal deaths and 1.7 million fewer newborn deaths from averting complications in pregnancy and childbirth.

    This morning, the US Supreme Court will hear oral arguments in Medina v. Planned Parenthood South Atlantic, the case challenging South Carolina’s efforts to block people enrolled in Medicaid from accessing health care at Planned Parenthood clinics.

    The Supreme Court will soon rule in the case US v. Skrmetti and the decision could undermine essential trans health care for young people nationwide.

    Guttmacher data show that young people and LGBTQ people already face unique obstacles when seeking sexual and reproductive health care, including abortion. Many of these barriers, such as a lack of financial resources or transportation, extend to gender-affirming care as well.

    Young trans people deserve to have their health needs recognized and met. We must keep fighting to ensure access to essential care for all trans youth.

    Fifteen years ago today, the Affordable Care Act was signed into law. Since then, it has improved the health and economic well-being of millions of people across the US in several ways.

    Major expansions to Medicaid under the ACA have dramatically reduced uninsured rates among women of reproductive age and increased access to sexual and reproductive health care. Medicaid's robust coverage of contraceptive care is vitally important for marginalized communities nationwide, including people living with low incomes, Black, Brown and other people of color and more.

    In addition, the ACA’s contraceptive coverage guarantee requires most private health insurance plans to cover the full range of contraceptive methods, services and counseling without out-of-pocket costs for patients.

    This has allowed millions of people to access their birth control method of choice, plan their families and protect their health and well-being! Learn more at the link in our bio.

    By expanding access to essential health care, both the contraceptive coverage guarantee and Medicaid have become crucial to reproductive autonomy. Policymakers must take action NOW to uphold the ACA and strengthen its protections.

    Medication abortion is a critical part of abortion care provision across the US.

    So it’s not surprising that anti-abortion policymakers are using every lever of government to target access to and availability of this safe and effective method.

    At the federal level, anti-abortion activists have advocated for rescinding FDA approval of mifepristone and enforcing the antiquated Comstock Act. At the state level, several lawmakers have enacted bans on providing medication abortion via telemedicine and imposed other restrictions that make it harder to get care.

    These attacks are grounded in fearmongering and disinformation, not evidence, and they may jeopardize access for all people—even those living in states where abortion is legal.

    Everyone deserves access to medication abortion. Lawmakers must protect and expand this access, so that all people can get and use their preferred method of abortion with safety and dignity.

    Last month, the Governor of Wyoming signed into law medically unnecessary and burdensome requirements, targeting the only procedural abortion clinic in the state, @wellspringhealthaccess. These restrictions are known as targeted regulation of abortion providers, or TRAP laws. Including Wyoming, 24 states have TRAP laws in effect.

    The state also enacted a restriction that requires medication abortion patients to undergo an ultrasound at least 48 hours before their abortion. This will force patients to travel even longer distances for basic reproductive health care, if they can even access abortion in the state anymore.

    Wellspring’s clinic has been forced to stop providing procedural abortions and has also paused offering medication abortion.

    In other news, an Arizona state court permanently blocked the state's 15-week abortion ban, which was not being enforced following the state's 2024 constitutional amendment and a stipulation from the state’s AG. Congrats to Arizonians who fought hard to protect abortion access in the state!

    Medication abortion accounted for the majority of abortions provided in nearly all US states without total abortion bans in 2023. This ranged from 95% in Wyoming and 84% in Montana to 44% in Washington, DC and 46% in Ohio.

    These new estimates from our Monthly Abortion Provision Study expand on Guttmacher’s previous finding that 63% of all US clinician-provided abortions in 2023 were medication abortions.

    The latest data are striking because the differences between states aren't based on any single characteristic.

    For instance, Wyoming, a large and rural state with low population density and restrictive abortion policies, had a very high proportion of medication abortion. On the other hand, medication abortion use was also high in Delaware (77%), which has high population and clinic density as well as protective abortion policies.

    As researcher Isabel DoCampo explains, these findings underscore that medication abortion is widely used across the country. No matter where someone lives, everyone deserves access to their preferred method of care, including abortion pills.

    Abortion providers are essential to protecting reproductive freedom in the US. Their role is especially critical as attacks on abortion access have escalated since the Dobbs decision and new threats are emerging under this administration.

    Providers nationwide go above and beyond to ensure that patients can make health care decisions with dignity and autonomy. In the face of coercive policies, stigma and threats, providers still show up to give compassionate and empathetic care.

    Guttmacher's data show that many patients are being forced to travel longer distances for care due to abortion bans and restrictions. Policymakers must step up to support abortion providers in local communities, including by ensuring financial support for clinics.

    Just last week, a Missouri judge temporarily blocked licensing regulations that made it difficult, if not impossible, for abortion clinics in the state to provide care.

    With these restrictions blocked, abortion care can resume in the state for the first time since Roe was overturned, nearly three years ago.

    Our map is up to date with Missouri’s category shifting from “Restrictive” to “Some Restrictions/Protections.”

    Our interactive map includes a new protective category after an update to our methodology. Now, you can see states whose shield law protections extend to telehealth.

    This policy aims to protect abortion providers from legal action—no matter where their patients are located.

    President Trump has unleashed several executive actions targeting trans rights and health care.

    These moves send a clear message: the US government is doubling down on restricting trans rights and access to basic health care. These orders aim to intimidate trans youth, their families and providers of gender-affirming care.

    If allowed to stand, the orders could:

    -Restrict access to gender-affirming care for transgender young people

    -Erase recognition of diverse gender identities

    -Bar trans people from military service

    Trans youth are already facing bans on gender-affirming care in 26 states. Now, the federal government is escalating its attacks. Organizations are fighting back against these harmful executive orders, but we know that they are causing chaos, confusion and fear and already impacting trans people’s access to health care.

    These policies are an affront to our rights, values and evidence-based health care.

    Following successful ballot measure campaigns in the 2024 election by organizers in New York and Colorado, abortion rights are protected in both state constitutions!

    In Colorado, Medicaid coverage for abortion care is also no longer restricted due to these protections.

    Colorado’s category on our interactive map now shifts from “Protective” to “Very Protective” and New York’s category remains at “Very Protective.”

    Medication abortion is a critical part of expanding abortion access. It accounted for 63% of all US abortions in 2023.

    But the incoming Trump-Vance administration will likely target medication abortion in several ways.

    This likely includes directing the FDA to restrict and or even eliminate access to mifepristone, used in the most common medication abortion regimen in the US. More on these threats at the link in bio.

    On December 20, a Missouri judge ruled that the state’s total abortion ban is unenforceable and blocked enforcement of several other abortion restrictions.

    Our interactive map is up to date with Missouri’s category moving from “Most Restrictive” to “Restrictive.”

    This is a huge step forward for state advocates, but the reality is that abortion care remains unavailable in Missouri for the time being due to licensing restrictions that make it extremely difficult for clinics to operate.

    The path forward will be challenging, but advocates are determined to fight for what Missourians want and deserve. These wins were possible because Missourians voted to protect abortion access in the state constitution!

    Abortion is an essential health care service and should be accessible for everyone, regardless of where one lives.

    But for 51 years, the Helms Amendment has interfered with access to safe abortion access in many countries. Helms prohibits all US-funded organizations from using US foreign assistance funding for abortion services.

    Helms is often conflated with another US policy, the global gag rule, which prevents foreign NGOs that receive US funding (or their subgrantees) from using their own, non-US funds for any abortion-related activities.

    These unjust policies threaten bodily autonomy worldwide and must end once and for all.

    In 2024, the gap between US states working to protect or restrict reproductive health and rights got wider. Our new analysis breaks down this year’s major state policy trends.

    While some states worked to protect and expand abortion, gender-affirming care, and access to contraception, others ramped up restrictions, fear, and criminalization.

    Our map shows abortion policies in effect in every US state. Here are the key stats as of December 13, 2024:

    -13 states are enforcing total abortion bans

    -7 states restricting abortion access at 18 weeks or earlier

    -23 states are enforcing bans on gender-affirming care

    -21 provisions were enacted to restrict abortion care access, while 39 provisions were enacted to protect or expand it.

    But the overall number of brick-and mortar indie clinics has decreased by nearly a third in the last decade, and 76 indies have been forced to close or stop providing abortions since Dobbs.

    Voters in 7 US states made history in November 2024 by enshrining abortion rights in their state constitutions—and policies are already shifting. Here’s the latest:

    Arizona: The state’s 15-week ban is not being enforced after a legal challenge brought by abortion rights groups and a stipulation signed by the Attorney General that the ban will remain blocked until 30 days after a final decision in the case. Some providers are offering care past 15 weeks, but restrictions remain, keeping AZ in the "Restrictive" category on our interactive map.

    Missouri: Constitutional protections are now in effect. While MO still has a total abortion ban and many other restrictions, advocates are working to expand access. The state remains at “Most Restrictive.”

    Maryland: Already a leader in protecting abortion rights, MD’s new constitutional protections move it to the "Most Protective" category!

    Iowa is the latest US state to demonstrate the harm that early gestational bans have on abortion access.

    After a 6-week ban went into effect on July 29, 2024, clinician-provided abortions in Iowa dropped 38% from the average in the first half of the year.

    Access to abortion pills via telemedicine in US states with shield law protections has likely softened an even steeper decline in abortions following the 6-week ban in Iowa and other states with similar bans.

    As our expert Amy Friedrich-Karnik explains, if access to this care is undermined at the federal level, the abortion access crisis would become even worse. Guttmacher experts will be closely monitoring as the Trump-Vance administration takes office in 2025.

    Young trans people deserve health care that allows them to make decisions about their lives and futures with dignity. This must include access to gender-affirming care and the full spectrum of reproductive health care, including abortion.

    In the US, state policy attacks on trans people have increased. In 2024, more than 35 bills were introduced to restrict access to, or outright ban, gender affirming care for young people.

    At the federal level, President-Elect Trump has laid out some of his plans to eliminate gender-affirming care access for minors, including by blocking physicians who provide this care from receiving federal Medicaid and Medicare funding.

    And the US Supreme Court will soon hear the case US v. Skrmetti, in which the state of Tennessee is attempting to restrict any gender-affirming care for minors. This is a dangerous and targeted attempt to undermine essential health care for trans youth.

    Young trans people deserve better. We must keep fighting to protect bodily autonomy for all.

    Contraception is a core component of reproductive autonomy and health care, and its use is nearly universal in the US. People use birth control for many reasons, including to avoid pregnancy, have healthier pregnancies or help time and space births, and more.

    That’s why contraception must be available to anyone who needs it, including trans and nonbinary communities, young people and those living with low incomes.

    The incoming Trump administration poses serious threats to contraceptive access. This will likely include renewed attacks on the Title X family planning program, which ensures affordable contraceptive care for millions of people. More on these threats at the link in bio.

    In the 2024 election, abortion won in 7 out of 10 US states where it appeared on the ballot. This is important progress toward protecting reproductive freedom for many more people.

    When these measures take effect, they will enshrine protections for abortion in state constitutions.

    The results also show that abortion continues to mobilize voters in the 7 states with successful ballot initiatives and even those where a measure didn't pass this year.

    For example, Florida’s abortion rights measure earned more than 57% support but didn’t overcome the state’s high margin (60%) for passing a constitutional amendment. The results send a clear message and speak to the efforts of state advocates and majority of voters who want abortion access for all.

    It's important to note that adding constitutional protection doesn’t automatically repeal bans or restrictions, and litigation is often necessary to overturn existing state laws.

    Now more than ever, policymakers must step up to protect and expand abortion rights and access.
