In the US, conservative members of Congress are advancing a reconciliation bill that slashes funding to Medicaid to help pay for tax cuts for the wealthy.
Medicaid is an essential source of health insurance that covered 16.2 million women in 2023, accounting for 21% of all US women aged 15-49.
Medicaid must be protected and strengthened so people can access the care they need and deserve, including critical reproductive health services.

In 2024, 155,000 people in the US traveled out of state for abortion care, accounting for 15% of all clinician-provided abortions in states without total bans.
This is a slight decline from 2023, when 169,700 people traveled across state lines for an abortion, but it's still close to double the number of patients who did so before the Dobbs decision (81,100 in 2020).
Travel to states with protective abortion policies has remained critical for people seeking care since Dobbs, especially for those living in states with total bans.
No ban or restriction will stop people from seeking the essential abortion care they need and deserve.

The Trump administration is withholding millions of dollars in funding for Title X, an attack that severely threatens access to affordable birth control and other essential care across the US.
On March 31, 16 Title X grantees received notice that the administration was “temporarily withholding” funding for many of their grants for the fiscal year.
Currently, 7 states have lost all Title X funding and another 15 have lost partial funding.
If this denial of funds becomes permanent, 834,000 people could lose access to sexual and reproductive health (SRH) care at Title X-supported health centers each year. This represents 30% of patients served annually by Title X, who may be forced to:
-Pay out of pocket for care
-Get care at a non-preferred health care provider
-Switch to a non-preferred method of contraception
-Forgo contraception and related care entirely
The entire point of Title X is to address disparities in access to contraception and other SRH care, including by serving people living with low incomes, Black, Latinx and other marginalized communities who already encounter systemic barriers to health care. This attack on the Title X network will disproportionately impact the very same communities.
Everyone deserves affordable access to essential SRH care.
The administration must immediately restore Title X funds, and Congress must hold the administration accountable for doing so.

Trans people deserve health care that allows them to make decisions about their lives and futures with dignity, including both gender-affirming care and abortion.
But access to these essential services remains under attack.
Last year, US states introduced more than 60 bills to restrict trans health care for young people. Twenty-four states currently ban this care and 11 of these states also have abortion bans in effect.
Anti-trans and anti-abortion politicians are following the same playbook—and they won't stop here. Targeting basic health care for trans youth is part of the long-term conservative agenda to restrict all aspects of reproductive health care.
As we mark Trans Visibility Day, we must keep fighting for a world where everyone can get the care they need and deserve, in their communities and on their own terms.

Fifteen years ago today, the Affordable Care Act was signed into law. Since then, it has improved the health and economic well-being of millions of people across the US in several ways.
Major expansions to Medicaid under the ACA have dramatically reduced uninsured rates among women of reproductive age and increased access to sexual and reproductive health care. Medicaid's robust coverage of contraceptive care is vitally important for marginalized communities nationwide, including people living with low incomes, Black, Brown and other people of color and more.
In addition, the ACA’s contraceptive coverage guarantee requires most private health insurance plans to cover the full range of contraceptive methods, services and counseling without out-of-pocket costs for patients.
This has allowed millions of people to access their birth control method of choice, plan their families and protect their health and well-being! Learn more at the link in our bio.
By expanding access to essential health care, both the contraceptive coverage guarantee and Medicaid have become crucial to reproductive autonomy. Policymakers must take action NOW to uphold the ACA and strengthen its protections.

Guttmacher data show that many LGBTQ+ individuals in the US seek and obtain abortions: As many as 16% of abortion patients don't identify as cisgender or heterosexual women.
But LGBTQ+ people face unique obstacles to abortion access. There are also widespread attacks on the health and rights of LGBTQ+ communities, including legislation targeting trans health care for young people.
Everyone should be able to get an abortion when they want or need it. We must break down the barriers that prevent LGBTQ+ people from getting the care they deserve.

So it’s not surprising that anti-abortion policymakers are using every lever of government to target access to and availability of this safe and effective method.
At the federal level, anti-abortion activists have advocated for rescinding FDA approval of mifepristone and enforcing the antiquated Comstock Act. At the state level, several lawmakers have enacted bans on providing medication abortion via telemedicine and imposed other restrictions that make it harder to get care.
These attacks are grounded in fearmongering and disinformation, not evidence, and they may jeopardize access for all people—even those living in states where abortion is legal.
Everyone deserves access to medication abortion. Lawmakers must protect and expand this access, so that all people can get and use their preferred method of abortion with safety and dignity.

The state also enacted a restriction that requires medication abortion patients to undergo an ultrasound at least 48 hours before their abortion. This will force patients to travel even longer distances for basic reproductive health care, if they can even access abortion in the state anymore.
Wellspring’s clinic has been forced to stop providing procedural abortions and has also paused offering medication abortion.
In other news, an Arizona state court permanently blocked the state's 15-week abortion ban, which was not being enforced following the state's 2024 constitutional amendment and a stipulation from the state’s AG. Congrats to Arizonians who fought hard to protect abortion access in the state!

These new estimates from our Monthly Abortion Provision Study expand on Guttmacher’s previous finding that 63% of all US clinician-provided abortions in 2023 were medication abortions.
The latest data are striking because the differences between states aren't based on any single characteristic.
For instance, Wyoming, a large and rural state with low population density and restrictive abortion policies, had a very high proportion of medication abortion. On the other hand, medication abortion use was also high in Delaware (77%), which has high population and clinic density as well as protective abortion policies.
As researcher Isabel DoCampo explains, these findings underscore that medication abortion is widely used across the country. No matter where someone lives, everyone deserves access to their preferred method of care, including abortion pills.

Fifty-two years ago, Roe v. Wade affirmed the right to abortion—a victory that changed the landscape of reproductive rights. But for Black women, Roe was the floor, not the ceiling. Despite the rights Roe safeguarded, many Black and brown women were denied access to the reproductive care we deserved due to reproductive care deserts or economic barriers to care.
The dismantling of #Roe exacerbated existing barriers to care and opened the door for 26 states to restrict or even criminalize abortion care. We know we need to restore the right to abortion, but what we need to build back must go further than #Roe to ensure everyone can make decisions for themselves, free from barriers and oppression.
This fight is far from over.

Last week, we were proud to join the Women’s Equality Center, @stateinnovation and the @reprocaucus in Washington, DC, where legislators from Latin America and the US discussed strategies for advancing reproductive justice and pushing back against anti-abortion policies.
Mexican lawmakers Mónica Silva Ruiz and Lilia Aguilar, and Argentine Senator Guadalupe Tagliaferri shared lessons from their work in the Green Wave movement, including navigating hostile environments and successfully legalizing abortion in their communities.
US state policymakers and advocates offered insights from their work in states such as Georgia, which ban abortion, and states like Maine that have expanded access.
The gathering represented a bold step forward in fostering collaboration across borders to protect reproductive justice and ensure abortion access for all. 💪💚

Trans youth are already facing bans on gender-affirming care in 26 states. Now, the federal government is escalating its attacks. Organizations are fighting back against these harmful executive orders, but we know that they are causing chaos, confusion and fear and already impacting trans people’s access to health care.

This year, having the facts on US abortion rights and access will be more crucial than ever before.
Guttmacher data show that state abortion bans have eroded abortion access, pushing essential health care farther away for many people nationwide.
Interstate travel for abortion has increased in the wake of Dobbs. Nearly 1 in 5 abortion patients traveled out of state for care in 2023, compared with 1 in 10 in 2020.
Despite bans, clinician-provided abortions have increased: In 2023, there were an estimated 1,032,140 abortions in states without total abortion bans—an 11% increase from 2020. This may be due to upticks in the availability of medication abortion via telehealth, financial support for abortion seekers and access to abortion care via protective state policies.
We also know that medication abortion remains a critical part of ensuring access for as many people as possible. In 2023, medication abortion accounted for 63% of abortions, up from 53% in 2020.
As 2025 continues, we’ll keep sharing evidence and insights from our Monthly Abortion Provision Study, including data for all of 2024.

#AbortionIsHealthCareOn December 20, a Missouri judge ruled that the state’s total abortion ban is unenforceable and blocked enforcement of several other abortion restrictions.
Our interactive map is up to date with Missouri’s category moving from “Most Restrictive” to “Restrictive.”
This is a huge step forward for state advocates, but the reality is that abortion care remains unavailable in Missouri for the time being due to licensing restrictions that make it extremely difficult for clinics to operate.
The path forward will be challenging, but advocates are determined to fight for what Missourians want and deserve. These wins were possible because Missourians voted to protect abortion access in the state constitution!

But for 51 years, the Helms Amendment has interfered with access to safe abortion access in many countries. Helms prohibits all US-funded organizations from using US foreign assistance funding for abortion services.
Helms is often conflated with another US policy, the global gag rule, which prevents foreign NGOs that receive US funding (or their subgrantees) from using their own, non-US funds for any abortion-related activities.
These unjust policies threaten bodily autonomy worldwide and must end once and for all.

#RepealHelmsIn 2024, the gap between US states working to protect or restrict reproductive health and rights got wider. Our new analysis breaks down this year’s major state policy trends.
While some states worked to protect and expand abortion, gender-affirming care, and access to contraception, others ramped up restrictions, fear, and criminalization.
Our map shows abortion policies in effect in every US state. Here are the key stats as of December 13, 2024:
-13 states are enforcing total abortion bans
-7 states restricting abortion access at 18 weeks or earlier
-23 states are enforcing bans on gender-affirming care
-21 provisions were enacted to restrict abortion care access, while 39 provisions were enacted to protect or expand it.

But the overall number of brick-and mortar indie clinics has decreased by nearly a third in the last decade, and 76 indies have been forced to close or stop providing abortions since Dobbs.

Arizona: The state’s 15-week ban is not being enforced after a legal challenge brought by abortion rights groups and a stipulation signed by the Attorney General that the ban will remain blocked until 30 days after a final decision in the case. Some providers are offering care past 15 weeks, but restrictions remain, keeping AZ in the "Restrictive" category on our interactive map.
Missouri: Constitutional protections are now in effect. While MO still has a total abortion ban and many other restrictions, advocates are working to expand access. The state remains at “Most Restrictive.”
Maryland: Already a leader in protecting abortion rights, MD’s new constitutional protections move it to the "Most Protective" category!

#ProtectTransYouth Iowa is the latest US state to demonstrate the harm that early gestational bans have on abortion access.
After a 6-week ban went into effect on July 29, 2024, clinician-provided abortions in Iowa dropped 38% from the average in the first half of the year.
Access to abortion pills via telemedicine in US states with shield law protections has likely softened an even steeper decline in abortions following the 6-week ban in Iowa and other states with similar bans.
As our expert Amy Friedrich-Karnik explains, if access to this care is undermined at the federal level, the abortion access crisis would become even worse. Guttmacher experts will be closely monitoring as the Trump-Vance administration takes office in 2025.
